I'm looking for an online service that would allow me to create a basic html form, paste it on my site, and collect the information in an online database that I don't have to set up or manage. I would then want to download or export the information to my computer.
I'm thinking of a service similar to an online email service like iContact, FeedBlitz, or Zookoda, but that is more flexible and customizable in the data you can collect.
Online database/data collection service?
